BENEFITS First Diagnosis | $250 if an individual receives a diagnosis of cancer during the first 30 days following the date of issue. Covered person may void coverage and receive a full refund of premiums paid. Insured will receive $10,000, $25,000 or $50,000, depending on which plan is chosen, if the first diagnosis of cancer occurs after the 30th day following the date of issue. Each covered person is limited to one benefit amount for first diagnosis of cancer. | Preventive Care | $75 for each covered person each year who has one of the following screening tests performed: mammogram, prostate specific antigen (PSA), flexible sigmoidoscopy, colonoscopy, ovarian cancer blood test (CA-125), pap smear, chest x-ray or hem occult stool specimen. (This benefit is available without a first diagnosis of cancer.) |
